What we cover
Work falls in five lanes:

A piece may touch more than one lane. The lead lane is the one that determines the topic-specific limitation, if any.
Fiction and composites
Fictional composites used to illustrate a workflow or documentation problem are labeled as such. They are not patient cases and are not presented as prevalence or outcome data.
Sources
Primary guidelines, statutes, regulations, and original research are preferred. Secondary summaries, news reports, company statements, and convenience abstracts are labeled when used. Review dates are stated when sources are checked for a given article.
Discussion signals—social posts, listserv threads, or hallway talk—may prompt a research question. They are not used to establish prevalence, causation, a standard of care, or a legal rule.
Endorsement
Naming a product, vendor, agency, or guideline is not an endorsement. The Psychiatric Record does not endorse a vendor or workflow unless that endorsement is expressly stated.

High-risk topics
Articles and tools on suicide, controlled substances, AI and privacy, coding, or legal interpretation carry a topic-specific limitation in addition to the short educational disclaimer. Examples already in use:
- Operations and payer clocks or maps: operational guidance, not billing or legal advice.
- AI-scribe tools: the tool does not approve a vendor or workflow.
- Suicide-risk worksheets: not a scoring instrument and does not determine disposition.
